Diagnosis: Avulsion fracture of the iliac apophysis.

Full history:

15-year-old girl with history of a fall on the left hip several months ago while playing softball. She has persistent pain. Radiographs and MRI were reportedly normal.

Radiopharmaceutical:

19.9 mCi Tc-99m MDP i.v.

Findings:

There is increased activity in the left iliac crest in the region of the iliac apophysis. Subsequent review of the patient's radiographs demonstrated widening of the distance between the left ilium and iliac apophysis most consistent with an avulsion injury.
